Old and busted: organ transplants. New hotness: 3D organ printing

Say you need a new trachea, a part of the body we’ve already managed to replicate using stem cells and successfully transplant to a human with late-stage tracheal cancer (I’m not making that up or exaggerating). With a 3D printer and a bunch of stem cell-saturated bio-ink, you might be able to just print that trachea on demand thanks to a new technique that lets you pass human embryonic stem cells (hESCs) through a printer nozzle without destroying them.

A team of researchers from Scotland announced Monday that they’d finally managed to get an inkjet-style printer to craft an organic 3D object. Not an actual organ (well, not yet), but these scientists claim they’ve been able to clear a crucial hurdle: getting hESCs, prized for their ability to become cells of any tissue type, to survive the printing process.

The solution involved rejiggering the way the inkjet-style 3D printer worked, specifically the printing valve, which had to be tweaked to ever-so-gently deposit blobs of hESCs in programmable patterns without compromising the viability and functionality of the cells themselves. The researchers figured out how to do this using two types of bio-inks as well as allow for independent control of the amount in each droplet (with considerable control granularity — down to less than five cells per droplet). The results of the experiment were just published in the bio-science print and online journal Biofabrication.

A legal spectre is haunting the NFL

In the wake of a vastly entertaining SuperBowl contest between the “San Francisco 50-1’ers”* and the “Baltimore Black Birds”*, Steve Chapman outlines the possibility that we won’t see too many more SuperBowl games:

Professional football is the most popular spectator sport in America, which is one reason yesterday’s Super Bowl was expected to draw 110 million viewers. With its famous athletes, storied franchises, and lucrative TV contracts, it’s an industry whose future appears limitless.

But football has a problem: the specter of mass brain damage among current and former players. So far, the steady trickle of disturbing revelations has had no apparent effect on ticket sales or TV ratings. What it has done, though, is more ominous: It has invited lawsuits.

If football falls into decline, it may not be the result of fans turning away, athletes avoiding it, or parents forbidding it. It may be from lawyers representing players who sustained chronic traumatic encephalopathy and expect to be compensated for the damage.

Walter Olson, a Cato Institute fellow, blogger (Overlawyered.com), and author of several books on liability, knows well how a tide of litigation can transform a landscape. And he has a bold prediction: “If we were to apply the same legal principles to football as we do to other industries, it would have to become extremely different, if not go out of business.”

“Seriously?” you may ask. A guy who made a good living engaging in high-speed collisions with 300-lb. blocks of granite can say he didn’t understand the risks involved? It may seem that case will be laughed out of court.

But Olson thinks not. “Courts have not been very friendly to this argument, particularly when something as grave as permanent brain damage is involved,” he told me. And it’s become apparent that while players were aware of the possibility of mangled knees, broken bones, and concussions, they didn’t grasp that repeated blows to the head could produce debilitating and irreversible mental harms.

Japanese finance minister: “elderly are an unnecessary drain on the country’s finances”

The Guardian reports on recent comments by the new finance minister in Japan:

Japan’s new government is barely a month old, and already one of its most senior members has insulted tens of millions of voters by suggesting that the elderly are an unnecessary drain on the country’s finances.

Taro Aso, the finance minister, said on Monday that the elderly should be allowed to “hurry up and die” to relieve pressure on the state to pay for their medical care.

“Heaven forbid if you are forced to live on when you want to die. I would wake up feeling increasingly bad knowing that [treatment] was all being paid for by the government,” he said during a meeting of the national council on social security reforms. “The problem won’t be solved unless you let them hurry up and die.”

Aso’s comments are likely to cause offence in Japan, where almost a quarter of the 128 million population is aged over 60. The proportion is forecast to rise to 40% over the next 50 years.

To compound the insult, he referred to elderly patients who are no longer able to feed themselves as “tube people”. The health and welfare ministry, he added, was “well aware that it costs several tens of millions of yen” a month to treat a single patient in the final stages of life.

Cost aside, caring for the elderly is a major challenge for Japan’s stretched social services. According to a report this week, the number of households receiving welfare, which include family members aged 65 or over, stood at more than 678,000, or about 40% of the total. The country is also tackling a rise in the number of people who die alone, most of whom are elderly. In 2010, 4.6 million elderly people lived alone, and the number who died at home soared 61% between 2003 and 2010, from 1,364 to 2,194, according to the bureau of social welfare and public health in Tokyo.

The government is planning to reduce welfare expenditure in its next budget, due to go into force this April, with details of the cuts expected within days.

Sadly, expect more of this kind of comment from hard-pressed governments as the baby boomers move out of work and into retirement.

The root problem with all self-help programs

In New York magazine, Kathryn Schulz explains why self-help programs are so popular … and why they are so difficult for most of us:

In The Age of Anxiety, W.H. Auden observed that we human beings never become something without pretending to be it first. The corollary is more prosaic but, regrettably, at least as true: We humans never become most of the things we pretend we will someday be. Nevertheless, last Monday, you and I and several billion other incorrigible optimists raised our glasses and toasted all the ways we will be different in 2013.

It’s easy to understand why we want to be different. We are twenty pounds overweight; we are $20,000 in debt; we can’t believe we slept with that guy; we can’t believe we didn’t. What’s harder to understand is why transforming ourselves is so difficult. Changing other people is notoriously hard; the prevailing wisdom on that one is Don’t hold your breath. But it’s not obvious why changing oneself should present any difficulty at all. And yet, demonstrably, it does.

The noted self-help guru Saint Augustine identified this problem back in the fourth century A.D. In his Confessions, he records an observation: “The mind gives an order to the body and is at once obeyed, but when it gives an order to itself, it is resisted.” I cannot improve upon Augustine’s insight, but I can update his examples. Say you want to be skinny. You’ve signed on with Weight Watchers, taken up Zumba, read everything from Michael Pollan to French Women Don’t Get Fat, and scrupulously recorded your every workout, footstep, and calorie on your iPhone. So whence the impulsive Oreo binge? [. . .]

This is where the cheerfully practical and accessible domain of self-help bumps up against one of the thorniest problems in all of science and philosophy. In the 1,600 years since Augustine left behind selfhood for sainthood, we’ve made very little empirical progress toward understanding our own inner workings. We have, however, developed an $11 billion industry dedicated to telling us how to improve our lives. Put those two facts together and you get a vexing question: Can self-help work if we have no idea how a self works?

Obesity meta-study challenged the media narrative

In sp!ked, Timandra Harkness explains why the publication of “Association of All-Cause Mortality With Overweight and Obesity Using Standard Body Mass Index Categories” in the Journal of the American Medical Association (JAMA) got such a strongly negative reaction from the media and various government spokespeople:

The reason this unassuming paper drew howls of outrage was the same as the reason the benefits of moderate alcohol intake are never noted without criticism: it spoils the headline health message that Fat is Bad.

Even worse, it blows the cover on the great myth — that an epidemic of Bad Fatness is sweeping the developed world. By including the dangerously obese, the innocuously tubby and the healthily plump in one category, ‘overweight including obese’, 60 per cent of the English population are labelled as potentially At Risk.

Being At Risk means these people need guidance and protection from their own vulnerable state, from the temptations of our obesogenic world and the frailties of their own sugar-addicted brains. At such a time of national peril, no measure is too extreme.

But less than a quarter of English adults are obese, according to new figures released just before Christmas, a fraction almost unchanged since 2007. And the ‘morbidly obese’ category — BMI over 40, the ones for whom it really might be worth shedding a few pounds, medically speaking — also remains steady since 2009 at 2.5 per cent of the UK population.

If only one in 40 of us is in significant weight-related danger, why do the other 97.5 per cent of us need to be protected by the state against sugary cereals and fizzy drinks? Could it be because only a few of us have fallen, but all of us are in peril? Weak, foolish and easily led astray, we need to be frightened back on to the right path. Thus Tam Fry, spokesman for the National Obesity Forum — who has called for children to be monitored from birth for signs of obesity — told the Independent: ‘If people read this and decide they are not going to die… they may find themselves lifelong dependents on medical treatment for problems affecting the heart, liver, kidney and pancreas — to name only a few.’

So there we have it. Those extra post-Christmas pounds aren’t going to kill you. If you’re approaching an age at which there’s any real prospect you will die, they probably have a tiny protective effect. But if you’re told the truth, suggest the obesity obsessives, you’ll gorge yourself into a disgusting ball of flab.

Don’t let your BMI scare you (too much)

A quick reminder that the Body Mass Index (BMI) is more a convenient mathematical trick than an actual healthy weight guideline:

In a finding that could undermine many New Year’s resolutions, a new government study shows that people who are overweight are less likely to die in any given period than people of normal weight. Even those who are moderately obese don’t have a higher-than-normal risk of dying.

Being substantially obese, based on measure called body mass index, or BMI, of 35 and higher, does raise the risk of death by 29%, researchers found.

But people with a BMI of 25 to 30 — who are considered overweight and make up more than 30% of the U.S. population — have a 6% lower risk of death than people whose BMI is in the normal range of 18.5 to 25, according to the study, being published Wednesday in the Journal of the American Medical Association.

People who had a BMI of 30 to 35 — considered the first stage of obesity — had a 5% lower risk of dying, but those figures weren’t considered statistically significant.

In other words, a few extra pounds are not going to threaten your life (a lot of extra pounds might). In the western world, few of us have the kind of jobs that require much in the way of physical exertion and we also have both relatively low food prices and much greater access to calorie-dense food. Our parents tended to have jobs that required more physical effort and their access to food was not as great as ours (they were less wealthy overall, and didn’t eat at restaurants or fast food joints as often as we do). Two otherwise positive trends that combine to produce a less-positive result on the scales.

More copper to fight superbugs

Brass and other copper-alloyed metals may have a bright future in doorknobs, handles, and other frequently handled surfaces due to a recent discovery about the metal’s ability to fight bacteria:

Researchers have discovered that copper and alloys made from the metal, including brass, can prevent antibiotic resistance in bacteria from spreading.

Plastic and stainless steel surfaces, which are now widely used in hospitals and public settings, allow bacteria to survive and spread when people touch them.

Even if the bacteria die, DNA that gives them resistance to antibiotics can survive and be passed on to other bacteria on these surfaces. Copper and brass, however, can kill the bacteria and also destroy this DNA.

Professor Bill Keevil, head of the microbiology group at Southampton University, said using copper on surfaces in public places and on public transport could dramatically cut the threat posed by superbugs.

In research published in the journal Molecular Genetics of Bacteria, Professor Keevil and his colleagues found that compared to stainless steel bacteria on copper surfaces bacterial DNA rapidly degraded at room temperature.

Professor Keevil added: “We live in this new world of stainless steel and plastic, but perhaps we should go back to using brass more instead.”

Tim Worstall points out that much of the stainless steel came in through health and safety regulation:

But isn’t this just great? All that modernity, all that ripping out of the old and replacement with futuristic design actually kills people?

It’s almost as fun as the discovery that the wooden chopping boards, which they made illegal, contain natural antibiotics which the plastic chopping boards, which they made compulsory, do not.

The Man from Whitehall really does not know best. And given that, can we hang them all from the Christmas tree please? It would usher in such a jolly New Year.

We’re at “peak farmland”

Matt Ridley on an interesting paper from Jesse Ausubel and Iddo Wernick of Rockefeller University, and Paul Waggoner of the Connecticut Agricultural Experiment Station:

Globally, the production of a given quantity of crop requires 65% less land than it did in 1961, thanks to fertilizers, tractors, pesticides, better varieties and other factors. Even corrected for different kinds of crops, the acreage required is falling at 2% a year.

In the U.S., the total corn yield and the total corn acreage tracked each other in lock step between 1870 and 1940-there was no change in average yield per acre. But between 1940 and 2010, corn production almost quintupled, while the acreage devoted to growing corn fell slightly. Similar divergences appeared later in other countries. Indian wheat production increased fivefold after 1970, while wheat acreage crept up by less than 1.5 times. Chinese corn production rose sevenfold over the same period while corn acreage merely doubled.

Yet the amount of farmland in the world was still rising until recently. The reason is that increased farm productivity has been matched by rising demand for food, driven by population growth and swelling affluence. But the effects of these trends are waning.

Even with these cautious assumptions, the researchers find that over the next 50 years people are likely to release from farming a land area “1½ times the size of Egypt, 2½ times the size of France, or 10 Iowas, and possibly multiples of this amount.”

Indeed, the authors find that this retreat from the land would have already begun but for one factor so lunatic that they cannot imagine it will not be reversed soon: biofuels. If the world had not decided to subsidize the growing of energy crops on 3.4% of arable land, then absolute declines in the acreage of arable land “would have begun during the last decade.” The prospect of “the restoration of vast acreages of Nature” is enticing for nature lovers.

The domestic terror: “I live with a son who is mentally ill. I love my son. But he terrifies me.”

Anarchist Soccer Mom has a story to tell. It’s neither pleasant nor uplifting, but it needs to be told:

I live with a son who is mentally ill. I love my son. But he terrifies me.

A few weeks ago, Michael pulled a knife and threatened to kill me and then himself after I asked him to return his overdue library books. His 7 and 9 year old siblings knew the safety plan — they ran to the car and locked the doors before I even asked them to. I managed to get the knife from Michael, then methodically collected all the sharp objects in the house into a single Tupperware container that now travels with me. Through it all, he continued to scream insults at me and threaten to kill or hurt me.

That conflict ended with three burly police officers and a paramedic wrestling my son onto a gurney for an expensive ambulance ride to the local emergency room. The mental hospital didn’t have any beds that day, and Michael calmed down nicely in the ER, so they sent us home with a prescription for Zyprexa and a follow-up visit with a local pediatric psychiatrist.

We still don’t know what’s wrong with Michael. Autism spectrum, ADHD, Oppositional Defiant or Intermittent Explosive Disorder have all been tossed around at various meetings with probation officers and social workers and counselors and teachers and school administrators. He’s been on a slew of antipsychotic and mood altering pharmaceuticals, a Russian novel of behavioral plans. Nothing seems to work.

When I asked my son’s social worker about my options, he said that the only thing I could do was to get Michael charged with a crime. “If he’s back in the system, they’ll create a paper trail,” he said. “That’s the only way you’re ever going to get anything done. No one will pay attention to you unless you’ve got charges.”

No one wants to send a 13-year old genius who loves Harry Potter and his snuggle animal collection to jail. But our society, with its stigma on mental illness and its broken healthcare system, does not provide us with other options. Then another tortured soul shoots up a fast food restaurant. A mall. A kindergarten classroom. And we wring our hands and say, “Something must be done.”

I agree that something must be done. It’s time for a meaningful, nation-wide conversation about mental health. That’s the only way our nation can ever truly heal.

God help me. God help Michael. God help us all.

We solve a worldwide problem … and replace it with a new worldwide problem

Malnutrition used to be one of the biggest problems facing the planet: except in the west, starvation was rarely more than a bad harvest away. Today, except in sub-Saharan Africa, that’s been replaced by obesity as a worldwide problem:

Here’s a good news story that warmed the cockles of my heart as I wolfed down my breakfast of chocolate croissant with extra-large latte — obesity is now more of a problem than starvation. As we report:

    With the exception of sub-Saharan Africa, eating too much is now a more serious risk to the health of populations than eating poorly, found the Global Burden of Disease study, published in a special edition of The Lancet.

    Across the world, there has been significant success in tackling malnutrition, with deaths down two-thirds since 1990 to less than a million by 2010.

    But increasing prosperity has led to expanding waistlines in countries from Colombia to Kazakhstan, as people eat more and get less everyday exercise.

Likewise the fact that humanity can not only feed billions of people, but feed them well enough to give many of them Type-2 diabetes, shouldn’t be considered a worry but, after the Moon landings, perhaps humanity’s greatest achievement yet. Yes, we’re all stuffing ourselves silly, but we evolved in an environment where food was scarce and fats were vital to our survival. The very fact that, despite numerous doomsayers, we continue to overcome our problems, is something we should be celebrating.

The revolution will not be revolutionary … soon

In the Globe and Mail, Timothy Caulfield explains that we need to be careful not to drink the “healthcare revolution” Kool-Aid:

It has been suggested that this technological advance will usher in a new health-care “revolution.” It will allow us, or so it’s promised, to individualize health-care treatments and preventive strategies — an approach often called “personalized medicine.” It will allow us to become fully aware of our genetic shortcomings and the diseases for which we’re at increased genetic risk, thus providing the impetuous to adopt healthier lifestyles.

But will having your personal genome available really revolutionize your health-care world? Will you be able to use this information to significantly improve your chances of avoiding the most common chronic diseases? Not likely.

Tangible benefits will be (and have been) achieved. But, for the most part, these advances are likely to be incremental in nature – which, history tells us, is the way scientific progress usually unfolds.

Why this “we are not in a revolution” message? Overselling the benefits of personal genomics can hurt the science, by creating unrealistic expectations, and distract us from other, more effective areas of health promotion.

The relationship between our genome and disease is far more complicated than originally anticipated. Indeed, the more we learn about the human genome, the less we seem to know. For example, results from a major international initiative to explore all the elements of our genome (the ENCODE project) found that, despite decades-old conventional wisdom that much of our genome was nothing but “junk DNA,” as much as 80 per cent of our genome likely has some biological function. This work hints that things are much more convoluted than expected. So much so that one of ENCODE’s lead researchers, Yale’s Mark Gerstein, was quoted as saying that it’s “like opening a wire closet and seeing a hairball of wires.”

“Big Food” is killing us!

At sp!ked, Rob Lyons debunks a recent video by Canadian anti-corporate activist Dr. Yoni Freedhoff:

This is a handy menu of food-related government intervention that is trotted out all the time by food crusaders everywhere. But before we get to those interventions, maybe we should ask how we got here in the first place.

First, food got cheaper while, on average, we’ve been generally getting richer. In particular, if America is anything to go by, we spent less as a proportion of income on meat and dairy products — surprisingly, spending on fruit and veg has been pretty constant — and more on processed foods and sweets. In other words, we bought convenience with the money we were saving.

Second, suppliers and retailers realised that as food got cheaper, the way to make money was to ‘add value’ — in other words, take basic ingredients and make them more convenient, more ‘fun’, more ‘premium’ or to appeal to some other psychological need. Yes, food manufacturers are as capable of bullshitting as anybody else with something to sell.

One of the other ways that suppliers add value is to make ‘healthy’ products. But who set up those health claims in the first place? It was the media, the medical profession and, most of all, governments. Who said we should be stuffing our faces with fruit to get our ‘five a day’? Who suggested that we get more omega-3s? Who said we should aim to eat low-fat diets? All of these ideas got the big official stamp of approval. And in the spirit of convenience, the food industry has made it easy, for better or for worse, to meet these official goals.

Moreover, what about the wild claims made for organic food? It has a completely spurious image as natural and wholesome, but study after study finds no consistent difference between organic foods and conventional foods — apart from the price. Yet it is often the most vociferously anti-Big Food campaigners, bloggers and ‘experts’ who push organic as the healthy alternative.

Rather than endless calls for regulations, bans and taxes — whose efficacy is doubtful but whose effect on personal autonomy would be substantial — it would be far better to recognise that any diet with some modicum of balance will be fine for most people, who will live to a greater age than their parents or grandparents, on average, no matter how much disapproved food they consume. Claims that any particular food is some dietary panacea should be treated with a large, metaphorical pinch of salt, whoever makes them, whether they are an evil mega corporation or the bloke behind the counter at the health-food shop.

Above all, a similarly healthy scepticism should be applied to crusading medics who want to scare us with the idea that Big Food is out to kill us and who encourage politicians to regulate what we eat.

Christine Jorgensen

The BBC has a retrospective on Christine Jorgensen, who started life as George Jorgensen, switching gender 60 years ago:

News of a pioneering sex change operation, one of the first involving both surgery and hormone therapy, was announced in 1952 — exactly 60 years ago this weekend.

“Ex-GI becomes blonde beauty!” screamed one headline as newspapers in the United States broke the news.

George Jorgensen, a quiet New Yorker, shocked a nation by returning from a trip to Denmark transformed into the glamorous Christine.

On her return to the US, Jorgensen was greeted with curiosity, fascination and respect by both the media and the public. There was relatively little hostility.

Hollywood embraced her. Theatre and film contracts began to roll in, she was invited to all the most glamorous parties and even crowned Woman of the Year by the Scandinavian Society in New York.

“I guess they all want to take a peek,” Jorgensen once said.

Throughout the 1960s and 70s she made a comfortable living, touring the country singing and doing impressions in her own show.

She was less successful in her personal life. Her first serious relationship broke down soon after their engagement. The next went as far as the register office, only for Jorgensen to be refused a marriage licence when she pulled out a man’s birth certificate.

Even in a disaster area, the bureaucrats stick to their role

I had to double-check the URL here to make sure this wasn’t a parody news item from The Onion:

Bobby Eustace, an 11-year veteran with the city’s fire department tells FoxNews.com that on Sunday he and his fellow firefighters from Ladder 27 in the Bronx were issued a notice of violation for not maintaining restaurant standards in a tent set up in Breezy Point, Queens, to feed victims and first responders.

“It’s just a little ridiculous. The inspector came up and asked if we were wearing hairnets. I told him, ‘We have helmets. This is a disaster area,’” Eustace told FoxNews.com. “Then he asked if we had gloves and thermometers [for food]. I said, “Yeah, we have rectal and oral. Which one do you want?’ He wasn’t amused.”

Eustace says that the Health Department worker then checked off a list of violations at the relief tent, including not having an HVAC system and fire extinguisher.

“He told us that he might come back to see if we fixed the violations. But what can we do? We are just going to keep going until a professional catering company can help take over,” Eustace said, adding that firefighters across the city together have been contributing about $800 a day out of their own pockets to feeding victims in areas hit hard by Sandy.

Reason.tv: Lance Armstrong Cheated to Win. Is that Wrong?

After months of bad press, the greatest competitive cyclist of all time has officially hit rock bottom: The Lance Armstrong Foundation has dropped the name of its eponymous creator and will now be known as the Livestrong Foundation.

Rest easy, Lance, it can’t get much — or is that any? — worse.

He may be a sanctimonious jerk whose doping denials are less convincing than a Lindsay Lohan rehab stint, but should he be pilloried for doing what all top cyclists — and increasingly, all of us — are doing: pursuing better living through chemistry?

Reason TV correspondent Kennedy defends performance-enhancing drugs from steroids to Viagra to that special memory pill we can’t remember the name of…
