“The VA really is a good example of a single-payer, socialized health system”

J.D. Tuccille on the Veterans Health Administration:

Just a couple of years ago, Paul Krugman pointed to the Veterans Health Administration (VHA) as a “huge policy success story, which offers important lessons for future health reform.” He gloated, “yes, this is ‘socialized medicine.'”

Similarly, a letter touted by Physicians for a National Health Program trumpeted “the success of 22 wealthy countries and our own Department of Veterans Affairs, which use single-payer systems to provide better care for more people at far less cost.”

How could a bloated government bureaucracy achieve such low-cost success? As we found out recently, it’s by quietly sticking veterans on a waiting list and putting off their treatment for months — sometimes until the patients are far too dead to need much in the way of expensive care. Which is to say, calling it a “success” is stretching the meaning of the word beyond recognition.

And, while the White House insists it learned from press reports about the secret waiting lists, Press Secretary Jay Carney acknowledges that the administration long knew about “the backlog and disability claims” that have accumulated in the VHA.

This should surprise nobody. Canada’s government-run single-payer health system has long suffered waiting times for care. The country’s Fraser Institute estimates [PDF] “the national median waiting time from specialist appointment to treatment increased from 9.3 weeks in 2010 to 9.5 weeks in 2011.”

Likewise, once famously social democratic Sweden has seen a rise in private health coverage in parallel to the state system because of long delays to receive care. “It’s quicker to get a colleague back to work if you have an operation in two weeks’ time rather than having to wait for a year,” privately insured Anna Norlander told Sveriges Radio.

So the VA really is a good example of a single-payer, socialized health system. Just not in the way that fans of that approach mean.

Even the Times thinks the latest climate scandal is damaging to the cause

The Times article titled “Scientists in Cover Up of ‘Damaging’ Climate View” is behind the usual paywall, but Steven Hayward has a longer excerpt that gets the key element across:

Research which heaped doubt on the rate of global warming was deliberately suppressed by scientists because it was “less than helpful” to their cause, it was claimed last night.

In an echo of the infamous “Climategate” scandal at the University of East Anglia, one of the world’s top academic journals rejected the work of five experts after a reviewer privately denounced it as “harmful”.

Lennart Bengtsson, a research fellow at the University of Reading and one of the authors of the study, said he suspected that intolerance of dissenting views on climate science was preventing his paper from being published. “The problem we now have in the climate community is that some scientists are mixing up their scientific role with that of a climate activist,” he added.

Professor Bengtsson’s paper challenged the finding of the UN’s Intergovernmental Panel on Climate Change (IPCC) that the global average temperature would rise by up to 4.5C if greenhouse gases in the atmosphere were allowed to double. It suggested that the climate might be much less sensitive to greenhouse gases than had been claimed by the IPCC in its report last September, and recommended that more work be carried out “to reduce the underlying uncertainty”.

Professor Bengtsson, the former director of the Max Planck Institute for Meteorology in Hamburg, said he accepted that emissions would increase the global average temperature but the key question was how quickly.

He added that it was “utterly unacceptable” to advise against publishing a paper on the ground that the findings might be used by climate sceptics to advance their arguments. “It is an indication of how science is gradually being influenced by political views. The reality hasn’t been keeping up with the [computer] models. Therefore, if people are proposing to do major changes to the world’s economic system we must have much more solid information.”

Russia, the United States, and the ISS – an opening for SpaceX

Motherboard‘s Jason Koebler says that Russia’s threat to ban companies from shipping rocket engines to the US represents a great chance for SpaceX to cash in:

Russia just announced plans to shut down the International Space Station in 2020, and prohibited companies in the country from selling engines to Lockheed Martin and Boeing for military launch purposes. If this is more than just posturing, there’s at least one takeaway: SpaceX is about to get paid.

It was just a month ago that NASA Administrator Charles Bolden said that Russia would never ban the United States from the ISS, and that his “contingency plan” for getting astronauts to and from the ISS was to continue working with Russia as normal until an American company is ready to fly manned flights to the ISS in roughly three years.

And that brings us to SpaceX. The company stands to gain greatly from both bits of news Rogozin announced today.

The company recently won (and then subsequently lost) an injunction to force the United States Air Force to compete for military satellite launch contracts. If Boeing and Lockheed Martin are blocked from buying Russian engines for their Atlas V and Delta IV from the other side (it’s worth noting that both countries have threatened to ban the companies from buying Russian engines), the Air Force once again has incentive to look at SpaceX as a legitimate option to launch military satellites.

The Air Force’s contract with the Boeing-Lockheed cooperative United Launch Alliance is worth roughly $70 billion through 2030 — SpaceX could potentially swoop in and take some of that amid the uncertainty.

Another area for freedom of choice – the “right to try”

Amity Shlaes talks about a movement to allow more freedom of choice, but in an unusual and tightly regulated sector:

For decades now the Food and Drug Administration has maintained an onerous and slow approval process that delays the debut of new drugs for fatal diseases, sometimes for years longer than the life span of the patients desperate to try them. Attorneys and scholars at the Goldwater Institute of Arizona have crafted legislation for the states that would allow terminally ill patients to try experimental drugs for cancer or degenerative neurological diseases earlier. These “Right to Try” bills are so scripted that they overcome the usual objection to delivery of such experimental drugs: safety. Under “Right to Try,” only drugs that have passed the crucial Phase 1 of FDA testing could be prescribed, thereby reducing the possibility of Thalidomide repeat. Second, only patients determined to have terminal cases would be eligible to purchase the drugs, making it harder to maintain that the drug will jeopardize their lives.

Representatives in Colorado, Louisiana, and Missouri approved the “Right to Try” measure unanimously. Citizens of Arizona will vote on the effort to circumvent the FDA process this fall.

Why the popularity? The phrase “Right to Try” appeals especially in a nation that senses all too well the reductions in freedom that come as the Affordable Care Act is implemented. The recent success of The Dallas Buyers’ Club, a film about a man who procured experimental drugs for AIDS patients, also fuels the “Right to Try” impulse. Some of the popularity comes from our culture of choice. In Colorado, where citizens have choice about abortion, and now the choice to use marijuana, they may also get what seems an elemental choice, that to try to save their own lives.

But of course “Right to Try” also sails because of the frustration of tragedy. Years ago a man named Frank Burroughs founded the Abigail Alliance after conventional options failed to cure his 21-year-old daughter’s cancer. Abigail’s oncologist tried to get Abigail newer drugs, Erbitux or Iressa from AstraZeneca, the company with which Pfizer hopes to merge. But the drugs were not available in time to save the girl. The Abigail Alliance is attempting on the federal level what Goldwater is trying for states: The federal bill’s name is the Compassionate Care Act. “Those waiting for FDA decisions, mainly dying patients and those who care for them, view the agency as a barrier,” co-founder Steve Walker explained simply. And who can disagree? Many of the supporters of “Right to Try” or the Abigail Alliance are businesspeople or scientists who are motivated to honor ones they have lost to illness; others are racing to save sick family who are still living. Yet others labor for patients in particular or science in general.

Climate change policies – “a massive conservative blind spot … matched by liberals’ tunnel vision”

Shikha Dalmia on the real reason the right fights against massive government action to fight climate change:

The decibel level in our national debate about global temperature went up several notches this week. The White House noisily released a report full of dire claims about the havoc manmade global warming is causing in America — and Republicans, equally noisily, denounced this as “liberal gloom and doom.”

The left has a deep ideological need to hype this issue, and the right to minimize it. And despite the deafening political noise on what ought to be a scientific matter, Americans must not be tempted to reach for their earplugs in disgust. After all, these ideological wars are how democracies sort out their differences.

The right’s chief commitment (which I share) is to free enterprise, property rights, and limited government that it sees as core to human progress. So when the market or other activities of individuals harm third parties or the environment, they look for solutions in these principles.

If overgrazing threatens a pasture, to use a classic example, the right’s answer is not top-down government diktats to ban or ration use. Rather, it is to divvy up the pasture, giving ownership to farmers — or privatizing the commons. The idea is that what individuals own, they protect; what they don’t, they abuse.

But there is no pure free market or property rights solution to global warming. There is no practical way to privatize the Earth’s atmosphere or divvy up pollution rights among the world’s seven billion inhabitants in 193 countries. This creates a planet-sized opening for the expansion of the regulatory state. Hence, right-wingers have an inherent need to resist the gloomy global warming narrative.

This is a massive conservative blind spot. But it is, in many ways, matched by liberals’ tunnel vision.

It is no secret that liberal commitment is less to promoting individual liberty and more to curbing capitalistic greed, which the left views as the great enemy of social justice and equality. At first blush, environmentalism and egalitarianism appear in conflict given that the environment is something of a luxury good that rich folks generally care about more than the poor.

Indeed, this conflict is why the 1960s New Left, driven primarily by humanistic concerns such as eradicating poverty and eliminating racism, shunned the emerging environmental movement for over a decade, according to University of Wisconsin’s Keith M. Woodhouse. Many in the New Left condemned the first Earth Day in 1970 as “the white liberal’s cop out” and believed that a preoccupation with overpopulation, for example, was “racist hysteria.”

In Forbes, Matthew Herper looks at how Novartis is transforming itself in an attempt to conquer cancer:

“I’ve been an oncologist for 20 years,” says Grupp, “and I have never, ever seen anything like this.” Emily has become the poster child for a radical new treatment that Novartis, the third-biggest drug company on the Forbes Global 2000, is making one of the top priorities in its $9.9 billion research and development budget.

“I’ve told the team that resources are not an issue. Speed is the issue,” says Novartis Chief Executive Joseph Jimenez, 54. “I want to hear what it takes to run this phase III trial and to get this to market. You’re talking about patients who are about to die. The pain of having to turn patients away is such that we are going as fast as we can and not letting resources get in the way.”

A successful trial would prove a milestone in the fight against the demon that has plagued living things since dinosaurs roamed the Earth. Coupled with the exploding capabilities of DNA-sequencing machines that can unlock the genetic code, recent drugs have delivered stunning results in lung cancer, melanoma and other deadly tumors, sometimes making them disappear entirely – albeit temporarily. Just last year the Food & Drug Administration approved nine targeted cancer drugs. It’s big business, too. According to data provider IMS Health, spending on oncology drugs was $91 billion last year, triple what it was in 2003.

But the developments at Penn point, tantalizingly, to something more, something that would rank among the great milestones in the history of mankind: a true cure. Of 25 children and 5 adults with Emily’s disease, ALL, 27 had a complete remission, in which cancer becomes undetectable. “It’s a stunning breakthrough,” says Sally Church, of drug development advisor Icarus Consultants. Says Crystal Mackall, who is developing similar treatments at the National Cancer Institute: “It really is a revolution. This is going to open the door for all sorts of cell-based and gene therapy for all kinds of disease because it’s going to demonstrate that it’s economically viable.”

David Harsanyi – The climate change debate is over

At The Federalist, David Harsanyi says the debate is over and the environmentalists have lost:

The truth is that even if Americans believed that scientists had seer-like abilities and the models were accurate, they would still be hesitant to embrace 19th century technology because they simply can’t afford it. Though I suspect most people instinctively understand that the environment has gotten better by almost every measure over the past 40 years, climate change activists ignore the massive benefits of carbon emitting fuels and technology that helps us become more productive and increasingly efficient.

Now, you can try and guilt trip everyone into compliance. You can batter them with distressing hypothetical scenarios. You can “educate” them on the issue from kindergarten onward, you can mainstream an array of Luddite ideas, you can browbeat society so they never utter a word of skepticism, but we still want to drive our cars everywhere. This is probably why over-the-top warnings and preposterous analogies have hit peak levels of absurdity.

And that’s saying something. Dr. John Holdren, Obama’s Science Czar, who was on media call for the National Climate Assessment release, once predicted global warming could cause the deaths of 1 billion people by 2020 and that sea levels would rise by 13 feet by the end of the century (not to mention, he co-authored a book with Paul Ehrlich in which he explained that “population-control laws, even including laws requiring compulsory abortion, could be sustained under the existing Constitution…”). Malthusians never admit they’re wrong, they simply push the apocalypse out a couple of decades. I just don’t think people believe them anymore.

Yes, when asked, Americans perfunctorily tell pollsters that climate change matters to them. A recent Pew Research Center poll found that 40 percent of Americans believe that climate change is a major threat. A Gallup poll survey found that around third of Americans personally worry about climate change. But when they’re not asked specifically about global warming, voters never bring the topic up. They’re most important concerns are the economy, jobs and debt. Though there is always strong support for the abstract idea of environmental regulation and “clean energy,” but when it comes some concrete policy it is nearly always unpopular. Few people want to stop the Keystone XL Pipeline. Few people support new emissions regulations. And I doubt another scaremongery study will change that reality.

Climate change and positive effects

Matt Ridley explains that according to the experts, it’s believed that ongoing climate change actually provides net benefits for most of this century:

Climate change has done more good than harm so far and is likely to continue doing so for most of this century. This is not some barmy, right-wing fantasy; it is the consensus of expert opinion. Yet almost nobody seems to know this. Whenever I make the point in public, I am told by those who are paid to insult anybody who departs from climate alarm that I have got it embarrassingly wrong, don’t know what I am talking about, must be referring to Britain only, rather than the world as a whole, and so forth.

At first, I thought this was just their usual bluster. But then I realised that they are genuinely unaware. Good news is no news, which is why the mainstream media largely ignores all studies showing net benefits of climate change. And academics have not exactly been keen to push such analysis forward. So here follows, for possibly the first time in history, an entire article in the national press on the net benefits of climate change.

There are many likely effects of climate change: positive and negative, economic and ecological, humanitarian and financial. And if you aggregate them all, the overall effect is positive today — and likely to stay positive until around 2080. That was the conclusion of Professor Richard Tol of Sussex University after he reviewed 14 different studies [PDF] of the effects of future climate trends.

To be precise, Prof Tol calculated that climate change would be beneficial up to 2.2˚C of warming from 2009 (when he wrote his paper). This means approximately 3˚C from pre-industrial levels, since about 0.8˚C of warming has happened in the last 150 years. The latest estimates of climate sensitivity suggest that such temperatures may not be reached till the end of the century — if at all. The Intergovernmental Panel on Climate Change, whose reports define the consensis, is sticking to older assumptions, however, which would mean net benefits till about 2080. Either way, it’s a long way off.

You can choose not to believe the studies Prof Tol has collated. Or you can say the net benefit is small (which it is), you can argue that the benefits have accrued more to rich countries than poor countries (which is true) or you can emphasise that after 2080 climate change would probably do net harm to the world (which may also be true). You can even say you do not trust the models involved (though they have proved more reliable than the temperature models). But what you cannot do is deny that this is the current consensus. If you wish to accept the consensus on temperature models, then you should accept the consensus on economic benefit.

Fukushima, radiation, and FUD

James Conca on a recent UN report that isn’t getting attention:

It’s always amazing when a United Nations report that has global ramifications comes out with little fanfare. The latest one states that no one will get cancer or die from radiation released from Fukushima, but the fear and overreaction is harming people (UNIS; UNSCEAR Fukushima; UNSCEAR A-68-46 [PDF]). This is what we’ve been saying for almost three years but it’s nice to see it officially acknowledged.

According to the report, drafted last year but only recently finalized by the U.N., “The doses to the general public, both those incurred during the first year and estimated for their lifetimes, are generally low or very low. No discernible increased incidence of radiation-related health effects are expected among exposed members of the public or their descendants. The most important health effect is on mental and social well-being, related to the enormous impact of the earthquake, tsunami and nuclear accident, and the fear and stigma related to the perceived risk of exposure to ionizing radiation. Effects such as depression and post-traumatic stress symptoms have already been reported.”

In addition, the report states, “Increased rates of detection of [thyroid] nodules, cysts and cancers have been observed during the first round of screening; however, these are to be expected in view of the high detection efficiency [using modern high-efficiency ultrasonography]. Data from similar screening protocols in areas not affected by the accident imply that the apparent increased rates of detection among children in Fukushima Prefecture are unrelated to radiation exposure.”

So the Japanese people can start eating their own food again, and moving back into areas contaminated with radiation levels similar to many areas of the world like Colorado and Brazil, which includes most of the exclusion zone. Only a few places shouldn’t be repopulated.

But if you want to continue feeling afraid, and want to make sure others keep being afraid, by all means ignore this report on Fukushima. But then you really can’t keep quoting previous UNSCEAR policy and application of LNT (the Linear No-Threshold dose hypothesis) to support more fear.

Note – LNT is a leftover Cold War ideology that states all radiation is bad, even the background radiation we are bathed in every day, even the 3,200 pCi of radiation in a bag of potato chips (yes, potato chips have the most radioactivity of any food, but they taste sooo good!).

Of course, if you’ve been actually following the events from three years back, this report will contain few surprises.

Fat’s negative health impact reconsidered

Hmm. Today seems to be health news day. In the Wall Street Journal, Nina Teicholz looks at the dubious science behind the saturated fat demonization we’ve all seen in so many health stories:

“Saturated fat does not cause heart disease” — or so concluded a big study published in March in the journal Annals of Internal Medicine. How could this be? The very cornerstone of dietary advice for generations has been that the saturated fats in butter, cheese and red meat should be avoided because they clog our arteries. For many diet-conscious Americans, it is simply second nature to opt for chicken over sirloin, canola oil over butter.

The new study’s conclusion shouldn’t surprise anyone familiar with modern nutritional science, however. The fact is, there has never been solid evidence for the idea that these fats cause disease. We only believe this to be the case because nutrition policy has been derailed over the past half-century by a mixture of personal ambition, bad science, politics and bias.

Our distrust of saturated fat can be traced back to the 1950s, to a man named Ancel Benjamin Keys, a scientist at the University of Minnesota. Dr. Keys was formidably persuasive and, through sheer force of will, rose to the top of the nutrition world — even gracing the cover of Time magazine — for relentlessly championing the idea that saturated fats raise cholesterol and, as a result, cause heart attacks.

Critics have pointed out that Dr. Keys violated several basic scientific norms in his study. For one, he didn’t choose countries randomly but instead selected only those likely to prove his beliefs, including Yugoslavia, Finland and Italy. Excluded were France, land of the famously healthy omelet eater, as well as other countries where people consumed a lot of fat yet didn’t suffer from high rates of heart disease, such as Switzerland, Sweden and West Germany. The study’s star subjects — upon whom much of our current understanding of the Mediterranean diet is based — were peasants from Crete, islanders who tilled their fields well into old age and who appeared to eat very little meat or cheese.

As it turns out, Dr. Keys visited Crete during an unrepresentative period of extreme hardship after World War II. Furthermore, he made the mistake of measuring the islanders’ diet partly during Lent, when they were forgoing meat and cheese. Dr. Keys therefore undercounted their consumption of saturated fat. Also, due to problems with the surveys, he ended up relying on data from just a few dozen men — far from the representative sample of 655 that he had initially selected. These flaws weren’t revealed until much later, in a 2002 paper by scientists investigating the work on Crete — but by then, the misimpression left by his erroneous data had become international dogma.

Take every salt report with a pinch of salt

Salt is bad for you, as we’ve been told for years and years. It raises your blood pressure and makes you more prone to strokes and other nasty health issues. Except … perhaps not:

The more salt we eat, the more water our body retains. This increases blood pressure, at least until our kidneys flush out the salt and water. Those who see salt as a problem believe that the effect on blood pressure is more lasting, and that if too much salt is ingested over a long period of time it will cause hypertension and perhaps death. A much-cited study carried out by America’s National Institutes of Health in 2001, called the DASH-sodium study, found that participants put on diets that were lower in sodium than the control group ended up with significantly lower blood pressure. This study forms the basis for many of the public-health pronouncements that demonise salt. America’s dietary guidelines, based on “a strong body of evidence”, put salt at the top of the list of things to avoid.

The body of evidence, though, is rather weaker than the American government lets on. The DASH study is one of many that have looked at the effects of salt intake on health. Others have failed to produce similar results. The English study mentioned above finds a correlation, but other factors — such as a simultaneous decline in smoking — seem more likely to account for the improved health outcomes. In 2011 two meta-analyses, which look at the results from many different studies, were published by the Cochrane Collaboration, a non-profit that reviews medical evidence. The first found that reducing salt intake leads to lower blood pressure, but concluded that there is “insufficient evidence” that this will lead to fewer premature deaths or a lower incidence of heart disease. The second concluded, rather simply, that “we do not know if low salt diets improve or worsen health outcomes.” The authors went on to say that “after more than 150 [randomised controlled trials] and 13 population studies without an obvious signal in favour of sodium reduction, another position could be to accept that such a signal may not exist.”

Some researchers go a step further, claiming that reducing salt intake actually increases a person’s risk of dying. The body needs some amount of sodium; if it gets too little the kidney secretes an enzyme called renin that can lead to hypertension. Some studies have found that low sodium levels were associated with increased risk of heart failure. Others suggest that a low sodium-to-potassium ratio may be the key to heart health. Much depends on the individual. The evidence is inconclusive, yet public-health officials have long presented the link between salt and heart disease as if it were fact. Such confidence is not warranted. There are plenty of reasons to avoid a full English breakfast, but salt may not be one of them.

SpaceX and the successful re-entry experiment

Amanda Wills talks about the most recent SpaceX achievement:

When SpaceX launched its Dragon supply mission to the International Space Station on April 18, it tried something revolutionary after the spacecraft was safely in orbit.

Behind the scenes, CEO Elon Musk and his team had been testing the reusability of this rocket. On that Friday, the team returned part of it to Earth for the first time in history. Once Dragon was in space, the first stage separated and re-entered Earth’s atmosphere. As the helium-filled rocket slowed, it extended four 25-foot-long landing legs and used its thrusters to briefly hover over the Atlantic Ocean before plopping down ever so gently onto its surface.

Musk and his team pulled it off — a huge feat considering that the chance of success was only around 30% to 40%. The SpaceX team recovered the raw video from the camera that was on board Falcon 9, and software engineers have spent the last week trying to repair the footage, which was taken just before splashdown.

The team was able to bring back the first stage. The rocket was clearly vertical — an important detail in testing reusable rockets — and the soft landing was successful. However, the weather wasn’t cooperative that day and the stage was destroyed by rough waves. Fortunately, Musk said his team was able to recover bits of the rocket.

I’m interested in life extension … I have no particular hankering to die any time soon, although I admit there is some truth in the aphorism “Many wish for immortality who don’t know how to spend a rainy Sunday afternoon”. Ray Kurzweil wants immortality, and he’s doing what he can to make that happen:

Ray Kurzweil — futurist, inventor, entrepreneur, bestselling author, and now, director of engineering at Google — wants to live forever. He’s working to make it happen. Kurzweil, whose many inventions include the first optical character recognition software (which transforms the written word into data) and the first text-to-speech synthesizer, spoke to Maclean’s for our annual Rethink issue about why we’re on the brink of a technological revolution — one that will improve our health and our lives, even after the robots outsmart us for good.

Q: You say we’re in the midst of a “grand transformation” in the field of medicine. What do you see happening today?

A: Biology is a software process. Our bodies are made up of trillions of cells, each governed by this process. You and I are walking around with outdated software running in our bodies, which evolved in a very different era. We each have a fat insulin receptor gene that says, “Hold on to every calorie.” That was a very good idea 10,000 years ago, when you worked all day to get a few calories; there were no refrigerators, so you stored them in your fat cells. I would like to tell my fat insulin receptor gene, “You don’t need to do that anymore,” and indeed that was done at the Joslin Diabetes Center. They turned off this gene, and the [lab mice] ate ravenously and remained slim. They didn’t get diabetes; they didn’t get heart disease. They lived 20 per cent longer. They’re working with a drug company to bring that to market.

Life expectancy was 20 a thousand years ago; 37, 200 years ago. We’re now able to reprogram health and medicine as software, and that [pace is] going to continue to accelerate. We’re treating biology, and by extension health and medicine, as an information technology. Our intuition about how progress will unfold is linear, but information technology progresses exponentially, not linearly. My Android phone is literally several billion times more powerful, per dollar, than the computer I used when I was a student. And it’s also 100,000 times smaller. We’ll do both of those things again in 25 years. It’ll be a billion times more powerful, and will be the size of a blood cell.
