A useful bit of perspective over Whinnygate

It’s reasonable to be concerned that your hamburger may once have raced in the Grand National, but worries about chemical contamination from the horse meat are almost certainly overblown. In fact, your health might be more at risk from the burger itself:

There is reasonable public outrage at possible criminal conspiracies to adulterate meat products with horsemeat, and additional concerns raised about the presence of the anti-inflammatory known as bute.

While not in any way questioning this concern about adulteration with a chemical compound, it is helpful to get a sense of magnitude. When bute was given as a human medicine, it was reported to be associated with a serious adverse reaction in 1 in 30,000 (over a whole course of treatment), but at a dose giving concentrations at least 4,000 times that arising from eating a diet of horse meat – see the excellent information from the Science Media Centre

So making all sorts of heroic assumptions about there being a linear-no-threshold response, we might very roughly assign a pro-rata risk of a serious event as 1 in 100,000,000 per burger.

QotD: When God sticks his nose into public health and taxation issues

It is not an original thought to say that public health crusaders often resemble religious zealots, but seldom is the comparison more literal than in the case of Mike Rayner, director of the British Heart Foundation Health Promotion Research Group.

So far, so mundane. Another illiberal battler against the free market with a heightened sense of his own importance and his nose in the trough. The only point of interest is that Mr Raynor is a Church of England priest who is guided by voices.

    In all of this I see a sacred dimension. You may not believe that I have heard God aright but I think God is calling me to work towards the introduction of soft-drink taxes in this country and I am looking forward to the day when General Synod debates the ethical issues surrounding this type of tax rather than some of the other issues that august body seems obsessed by.

Golly. Where to begin? On a theological note, I do wonder whether Jesus would really be in favour of a deeply regressive stealth tax that would take from the poor to give to the rich. Perhaps the reason the General Synod does not debate tax policy is because they recall the old “render under to Caesar…” message and realise that it’s none of their business.

If we weren’t already sceptical about the documents coming from Mr Rayner’s team of would-be policy-makers, the fact that its director believes that God has told him to bring about a fat tax in this land should be enough to make us suspect that a tiny bit of research bias might have crept into his work. Considering that the Almighty has approved of the policy, what are the chances of his loyal servant producing evidence that would question its efficacy?

Everything is cyclical — Baby Boom to Baby Bust

In the Wall Street Journal, Jonathan Last looks at the demographic changes on tap for the United States as the fertility rate continues to drop below replacement:

The fertility rate is the number of children an average woman bears over the course of her life. The replacement rate is 2.1. If the average woman has more children than that, population grows. Fewer, and it contracts. Today, America’s total fertility rate is 1.93, according to the latest figures from the Centers for Disease Control and Prevention; it hasn’t been above the replacement rate in a sustained way since the early 1970s.

The nation’s falling fertility rate underlies many of our most difficult problems. Once a country’s fertility rate falls consistently below replacement, its age profile begins to shift. You get more old people than young people. And eventually, as the bloated cohort of old people dies off, population begins to contract. This dual problem — a population that is disproportionately old and shrinking overall — has enormous economic, political and cultural consequences.

For two generations we’ve been lectured about the dangers of overpopulation. But the conventional wisdom on this issue is wrong, twice. First, global population growth is slowing to a halt and will begin to shrink within 60 years. Second, as the work of economists Esther Boserups and Julian Simon demonstrated, growing populations lead to increased innovation and conservation. Think about it: Since 1970, commodity prices have continued to fall and America’s environment has become much cleaner and more sustainable — even though our population has increased by more than 50%. Human ingenuity, it turns out, is the most precious resource.

Low-fertility societies don’t innovate because their incentives for consumption tilt overwhelmingly toward health care. They don’t invest aggressively because, with the average age skewing higher, capital shifts to preserving and extending life and then begins drawing down. They cannot sustain social-security programs because they don’t have enough workers to pay for the retirees. They cannot project power because they lack the money to pay for defense and the military-age manpower to serve in their armed forces.

Update: Kelly McParland on the plight of some older workers: “If they’d never worked at all, and gotten by on social assistance, they might still have a financial lifeline.”

It would be cruel (and maybe unfair) to say they made their own beds, but it remains the fact that a great deal of the trouble they face results from the refusal to brook a more prudent approach to public finances for so many years. Programs that were unaffordable were pushed through time and again, paid for by more and more borrowing. When crises developed, the borrowing increased while spending was only rarely curtailed. The curse of deficit financing is its snowball effect: annual shortfalls pile up, pushing up the carrying costs, creating a self-perpetuating ever-expanding spending crisis. When a recession inevitably arrives, there are no reserves to deal with it, and even more borrowing ensues.

After so many decades of pretending it could go on forever, without there being a reckoning, the generation that created it is discovering how wrong they were. Not only is it destroying the retirement dreams of so many near-seniors, it’s preparing a poisoned legacy to hand to the next generation, and perhaps the one after that, unless they recognize the need for greater discipline and finally accept the pain that will necessary to put the process back on a sustainable track.

Canada is fortunate that it faced up to its debt crisis 15 years ago and is still benefiting from that fact, but the public memory is short and there will always be pressure to turn a blind eye to debt, and legislate for today. No wonder people get more conservative as they get older. They understand the price that has to be paid for putting costs off to tomorrow.

Obesity meta-study challenged the media narrative

In sp!ked, Timandra Harkness explains why the publication of “Association of All-Cause Mortality With Overweight and Obesity Using Standard Body Mass Index Categories” in the Journal of the American Medical Association (JAMA) got such a strongly negative reaction from the media and various government spokespeople:

The reason this unassuming paper drew howls of outrage was the same as the reason the benefits of moderate alcohol intake are never noted without criticism: it spoils the headline health message that Fat is Bad.

Even worse, it blows the cover on the great myth — that an epidemic of Bad Fatness is sweeping the developed world. By including the dangerously obese, the innocuously tubby and the healthily plump in one category, ‘overweight including obese’, 60 per cent of the English population are labelled as potentially At Risk.

Being At Risk means these people need guidance and protection from their own vulnerable state, from the temptations of our obesogenic world and the frailties of their own sugar-addicted brains. At such a time of national peril, no measure is too extreme.

But less than a quarter of English adults are obese, according to new figures released just before Christmas, a fraction almost unchanged since 2007. And the ‘morbidly obese’ category — BMI over 40, the ones for whom it really might be worth shedding a few pounds, medically speaking — also remains steady since 2009 at 2.5 per cent of the UK population.

If only one in 40 of us is in significant weight-related danger, why do the other 97.5 per cent of us need to be protected by the state against sugary cereals and fizzy drinks? Could it be because only a few of us have fallen, but all of us are in peril? Weak, foolish and easily led astray, we need to be frightened back on to the right path. Thus Tam Fry, spokesman for the National Obesity Forum — who has called for children to be monitored from birth for signs of obesity — told the Independent: ‘If people read this and decide they are not going to die… they may find themselves lifelong dependents on medical treatment for problems affecting the heart, liver, kidney and pancreas — to name only a few.’

So there we have it. Those extra post-Christmas pounds aren’t going to kill you. If you’re approaching an age at which there’s any real prospect you will die, they probably have a tiny protective effect. But if you’re told the truth, suggest the obesity obsessives, you’ll gorge yourself into a disgusting ball of flab.

More copper to fight superbugs

Brass and other copper-alloyed metals may have a bright future in doorknobs, handles, and other frequently handled surfaces due to a recent discovery about the metal’s ability to fight bacteria:

Researchers have discovered that copper and alloys made from the metal, including brass, can prevent antibiotic resistance in bacteria from spreading.

Plastic and stainless steel surfaces, which are now widely used in hospitals and public settings, allow bacteria to survive and spread when people touch them.

Even if the bacteria die, DNA that gives them resistance to antibiotics can survive and be passed on to other bacteria on these surfaces. Copper and brass, however, can kill the bacteria and also destroy this DNA.

Professor Bill Keevil, head of the microbiology group at Southampton University, said using copper on surfaces in public places and on public transport could dramatically cut the threat posed by superbugs.

In research published in the journal Molecular Genetics of Bacteria, Professor Keevil and his colleagues found that compared to stainless steel bacteria on copper surfaces bacterial DNA rapidly degraded at room temperature.

Professor Keevil added: “We live in this new world of stainless steel and plastic, but perhaps we should go back to using brass more instead.”

Tim Worstall points out that much of the stainless steel came in through health and safety regulation:

But isn’t this just great? All that modernity, all that ripping out of the old and replacement with futuristic design actually kills people?

It’s almost as fun as the discovery that the wooden chopping boards, which they made illegal, contain natural antibiotics which the plastic chopping boards, which they made compulsory, do not.

The Man from Whitehall really does not know best. And given that, can we hang them all from the Christmas tree please? It would usher in such a jolly New Year.

Even in a disaster area, the bureaucrats stick to their role

I had to double-check the URL here to make sure this wasn’t a parody news item from The Onion:

Bobby Eustace, an 11-year veteran with the city’s fire department tells FoxNews.com that on Sunday he and his fellow firefighters from Ladder 27 in the Bronx were issued a notice of violation for not maintaining restaurant standards in a tent set up in Breezy Point, Queens, to feed victims and first responders.

“It’s just a little ridiculous. The inspector came up and asked if we were wearing hairnets. I told him, ‘We have helmets. This is a disaster area,’” Eustace told FoxNews.com. “Then he asked if we had gloves and thermometers [for food]. I said, “Yeah, we have rectal and oral. Which one do you want?’ He wasn’t amused.”

Eustace says that the Health Department worker then checked off a list of violations at the relief tent, including not having an HVAC system and fire extinguisher.

“He told us that he might come back to see if we fixed the violations. But what can we do? We are just going to keep going until a professional catering company can help take over,” Eustace said, adding that firefighters across the city together have been contributing about $800 a day out of their own pockets to feeding victims in areas hit hard by Sandy.

Regulating the size of soft drinks won’t solve the obesity problem, but will infringe on individual rights

At Reason, Baylen Linnekin explains that even if all the claims about the nutritional evils of sweetened soft drinks are completely true, regulations will not actually make much difference:

As an opponent of increased regulations, I find these latter scientific points noteworthy. But I also believe that even if sugar-sweetened drinks turn out to be virtually everything their opponents claim, people still have a right to buy and drink these beverages — just as much, as I argued in a recent Bloggingheads debate, as they have a right to buy a Big Mac. After all, we don’t have a right to free speech or to travel from one state to another because speech or travel has been proven by the scientific community to promote good health.

But suppose, for the sake of argument, I was to take at face value the assertions of those who claim the NEJM studies justify some combination of sugary drink taxes and bans.

There is still this problem: The solutions these advocates propose won’t likely solve the problem of obesity. For example, studies have suggested taxes will have little or no impact on obesity. And not one person has (to the best of my knowledge) even attempted to argue that soda bans would have any specific impact, either — unless one counts “sending a message” or “creating a debate” as conditions precedent to weight loss.

There is also the issue of a genetic predisposition, which again is one finding of the studies. Many people are genetically predisposed to certain food allergies — including soy, dairy, gluten, nuts, and seafood — and food intolerances. I have never seen a researcher or AP journalist like Marchione argue seriously that the widespread impact of food allergies “adds weight to the push for taxes” on wheat, tofu, and shrimp. Yet if one were to buy the argument of those calling for taxes and bans to combat consumption of sugary drinks in light of the NEJM studies, one would have to accept the idea of taxing society writ large based largely on the outcomes of what these researchers argue is a genetic condition.

Jacob Sullum on the legacy of Thomas Szasz

Jacob Sullum‘s post on the influence the late Thomas Szasz had and continues to have:

The idea that psychiatry became scientifically rigorous soon after Szasz first likened it to alchemy and astrology is hard to take seriously. After all, it was not until 1973 that the American Psychiatric Association (APA) stopped calling homosexuality a mental disorder.

More often, psychiatry has expanded its domain. Today it encompasses myriad sins and foibles, including smoking, overeating, gambling, shoplifting, sexual promiscuity, pederasty, rambunctiousness, inattentiveness, social awkwardness, anxiety, sadness, and political extremism. If it can be described, it can be diagnosed, but only if the APA says so.

For more than half a century, Szasz stubbornly highlighted the hazards of joining such a fuzzy, subjective concept with the force of law through involuntary treatment, the insanity defense, and other psychiatrically informed policies.

Consider “sexually violent predators,” who are convicted and imprisoned based on the premise that they could have restrained themselves but failed to do so, then committed to mental hospitals after completing their sentences based on the premise that they suffer from irresistible urges and therefore pose an intolerable threat to public safety. From a Szaszian perspective, this incoherent theory is a cover for what is really going on: the retroactive enhancement of duly imposed sentences by politicians who decided certain criminals were getting off too lightly — a policy so plainly contrary to due process and the rule of law that it had to be dressed up in quasi-medical, pseudoscientific justifications.

Szasz specialized in puncturing such pretensions. He relentlessly attacked the “therapeutic state,” the unhealthy alliance of medicine and government that blesses all sorts of unjustified limits on liberty, ranging from the mandatory prescription system to laws against suicide.

Did China peak in 2008?

Walter Russell Mead wonders if the Chinese economy actually hit its peak in 2008 and will not be able to get back to that level of performance:

According to The Diplomat, the long term outlook is even more depressing. China will have to confront a series of structural challenges if it is to continue to achieve the kind of dynamic growth that lifted the country from economic backwater to emerging great power in just three decades.

The most obvious challenge is demographics. A RAND study observed that the proportion of the Chinese population of working age peaked in 2011 and began slowing this year. The share of the elderly population is rising. Healthcare and pension costs will soar as a result. So will labor costs. Investment and savings will diminish. In short, China may face the prospect, unknown in human history, of growing old before it gets rich.

The environment presents another dilemma. Like many rapidly industrializing economies, China sacrificed environmental protection at the altar of economic growth. But the effects of this approach have taken a toll: already, argues The Diplomat, ”Water and air pollution today cause 750,000 premature deaths and around 8 percent of GDP.” And as Via Meadia recently pointed out, the political costs of this approach are starting to mount as well. An outbreak of NIMBYism has forced many local officials to cancel major industrial projects as ordinary Chinese citizens demand an end to environmentally unsound development.

Of greater concern is that China has backed away from market reforms in the last decade and embraced a version of “state capitalism” that emphasizes the state far more than it does capitalism. But as state-run entities have become more powerful, their political backers — and financial beneficiaries — have an even greater stake in blocking attempts at reform.

Quebec continues to strive to exclude Anglophones

A real head-scratcher in the Montreal Gazette: telephone staff at the Régie de l’assurance maladie du Québec (the Quebec health insurance board) are now required to assess callers’ language skills to determine if they actually require service in English.

Where before callers were given the option of service in English or French by way of a simple touch of the telephone keypad, it has now become more complicated. Now some people who would prefer to have the information given in English could be denied the service on the basis of a subjective judgment of their ability to speak French.

The way it works now is that calls to RAMQ are answered automatically in French, and callers are told that the agency first communicates with its clientele in French. Only after half a minute of silence is it mentioned that service in English is available by pressing 9. But wait: that doesn’t automatically get you service in English.

What it gets you is another recorded message, this time in English, informing you once more that the board prefers to deal with customers in French. The agents who subsequently come on the line do not speak English right away, even though the language of service chosen is English. No, the agents proceed in French, and are then required by the new policy to “use their judgment” to determine whether the caller speaks French well enough to be able to hold a conversation about health in French rather than English. Only if the caller fails that test will service in English be forthcoming.

Toronto edges cautiously toward allowing wider range of “street food”

Matt Gurney in the National Post on Toronto’s inch-by-glacial-inch move toward allowing a bit more variety in the foods street vendors can sell:

Last week, Toronto City Council approved hot dog vendors to sell an expanded variety of foods. The expanded list is still far from expansive. Veggie sticks, fruit salads and bagels with individually packaged butters are about the extent of the street food revolution in Toronto. Even these baby steps are progress, though — they follow the total failure of Toronto’s A La Cart program, which tried to expand the city’s food options to include more “ethnic” fare. The program, which should go down in history as the most botched effort the city has ever made, is Prosecution Exhibit A for those who believe that governments only exist to screw up things that really aren’t all that complicated.

But the city’s concern about street food, though overwrought and frankly embarrassing, at least comes from an honest place — concerns about spoiled food or improper preparation hurting public health. But Toronto has always missed the point. The public is protected when governments monitor outcomes and harshly punish failures, not seek to control process. Health inspections are an entirely reasonable part of the government’s job, with street food as much as any industry. And it seems that Toronto, while fretting about what food vendors might be doing wrong, hasn’t exactly been doing a bang-up job of its own responsibilities.

Hard though it is to imagine, other cities — even other Canadian cities — somehow manage to have all sorts of tasty treats for sale by food trucks, carts, and temporary kiosks without civilization crumbling.

BMI: Badly Misleading Indicator

Lewis Page explains why being too thin is a bad idea health-wise:

Yet another study has shown that the so-called “obesity” epidemic sweeping the wealthy nations of the world has been massively over-hyped, as new results show that is is far more dangerous to be assessed as “underweight” than it is to be assessed even as “severely obese” — let alone merely “obese” or “overweight”.

“There is currently a widespread belief that any degree of overweight or obesity increases the risk of death, however our findings suggest this may not be the case,” says health prof Anthony Jerant, lead author of the study. “In the six-year timeframe of our evaluation, we found that only severe obesity was associated with an increased risk of death.”

Most statistics in this field are still based on the now widely discredited Body Mass Index (BMI) system, under which people are assessed as “underweight”, “normal”, “overweight”, “obese” or “severely obese”. BMI, devised in the early 19th century by an obscure Belgian sociologist without medical qualifications, copes poorly with increases in height as it assumes the human body will scale up in mass in proportion to the square of height — which doesn’t allow for the fact that bodies are three dimensional — and further fails to allow for the greater cross-sectional area needed in supporting structures to carry increasing weights.

“We do not know if low salt diets improve or worsen health outcomes”

You’ve undoubtedly heard lots of recommendations to reduce the salt in your diet, right? The western diet — especially the North American variant — has “too much salt”, and it’s killing us. At least, that’s what has been drummed into our heads for the last twenty years or more. The problem is that is may not actually be true, and in fact may create other health issues:

Although researchers quietly acknowledged that the data were “inconclusive and contradictory” or “inconsistent and contradictory” — two quotes from the cardiologist Jeremiah Stamler, a leading proponent of the eat-less-salt campaign, in 1967 and 1981 — publicly, the link between salt and blood pressure was upgraded from hypothesis to fact.

When researchers have looked at all the relevant trials and tried to make sense of them, they’ve continued to support Dr. Stamler’s “inconsistent and contradictory” assessment. Last year, two such “meta-analyses” were published by the Cochrane Collaboration, an international nonprofit organization founded to conduct unbiased reviews of medical evidence. The first of the two reviews concluded that cutting back “the amount of salt eaten reduces blood pressure, but there is insufficient evidence to confirm the predicted reductions in people dying prematurely or suffering cardiovascular disease.” The second concluded that “we do not know if low salt diets improve or worsen health outcomes.”

The idea that eating less salt can worsen health outcomes may sound bizarre, but it also has biological plausibility and is celebrating its 40th anniversary this year, too. A 1972 paper in The New England Journal of Medicine reported that the less salt people ate, the higher their levels of a substance secreted by the kidneys, called renin, which set off a physiological cascade of events that seemed to end with an increased risk of heart disease. In this scenario: eat less salt, secrete more renin, get heart disease, die prematurely.

With nearly everyone focused on the supposed benefits of salt restriction, little research was done to look at the potential dangers. But four years ago, Italian researchers began publishing the results from a series of clinical trials, all of which reported that, among patients with heart failure, reducing salt consumption increased the risk of death.

Reason.tv: Obamacare goes to the Supreme Court

Does the fate of a federal government with limited powers rest in the hands of Supreme Court Justice Antonin Scalia? And if so, will he rule against broad federal powers (as he did in the Gonzales case) or in favor of the feds’ right to regulate just about anything (as he did in the Raich case)?

The Supreme Court case over The Affordable Care Act, a.k.a. Obamacare, “is certainly the most important case on the reach of federal power in 50 years” says attorney and legal scholar Timothy Sandefur of the Pacific Legal Foundation. “The constitutional principle of where is the line drawn on federal power — that’s a matter that our children and grandchildren will have to live with.”

The ruling will come sometime in early June, predicts Sandefur, who tells Reason.tv that the Affordable Care Act raises multiple constitutional issues: Can part of the law be struck down and other upheld? Is the “individual mandate,” which forces all Americans to purchase insurance as a condition of simply being alive, legal? Does the law’s massive expansion of Medicaid shred the right of states to govern their own finances?

Ireland’s septic protest

Elizabeth sent me a link to this Independent.ie article which allowed Lise Hand to dig deep into the Irish septic tank issue while managing not to get too potty-mouthed:

These doughty lads of the West weren’t messing about with a bit of chanting and poster-waving in the manner of an, ahem, bog-standard protest outside Leinster House. Not a bit of it, having driven since dawn in buses up from the corners of Galway, the attitude was, when we’re out, we’re out.

And so the Charge of the Septic Tank Brigade to the gates of Leinster was a colourful affair. They had brought a toilet with them and all, as a pertinent prop to illustrate their admanatine opposition to the introduction of a €50 septic-tank registration charge — a charge which affects rural Ireland, as it’s being imposed on almost half a million households who are not part of a public-sewage scheme.

What’s more, if any tanks fail an inspection, householders will be obliged to upgrade or replace them, which could cost thousands of euro.

And so, the several hundred men (and a few women) from the West were in fighting form on Kildare Street yesterday afternoon. And along with the toilet — which proved a handy seat for the protest’s organiser, Padraig ‘An Tailliura’ O’Conghaola from Rossaveal who was minding the megaphone and trying to keep a bit of order on proceedings.

There was an impressive array of giant paintings on black banners, tastefully depicting images such as sunsets and sailboats and a puzzled-looking lassie sitting on a toilet.

And there was quite a smorgasbord of slogans being waved about: from Winston Churchill’s observation, “We contend that for a nation to tax itself into prosperity is like a man standing in a bucket trying to lift himself up by the handle”; to more earthy exhortations, such as: “Septic Tank Charges are A Pain in the Hole”; and the bi-lingual “‘Cac’ Hogan RIP — Ireland’s Saddam Hussein”; to the pithy enjoinder, “Get A Grip — Stand Up to Europe”.
