Autism

Sandy Starr refutes the notion that “we are all on the autism spectrum now”:

Is autism a disorder? Is autism an identity? If you had asked me these questions a few years ago, before I became involved with the Autism Ethics Group at King’s College London, then my answer would have been a clear ‘yes’ and ‘no’ respectively. Clearly, autism is most usefully understood as a disorder. And clearly, it is not useful to understand autism as an identity.

If you were to ask me the same two questions today, then I would say exactly the same thing.

The whole concept of autism originates in psychopathology. Hans Asperger (after whom Asperger’s syndrome is named) talked about ‘autistic psychopathy’. Autism is found in the Diagnostic and Statistical Manual of Mental Disorders (DSM). And yet we now seem uneasy about the characterisation of autism as a disorder. Why?

For one thing, a disorder implies a lack of normal or typical function. The increasing numbers of people who are thought to warrant a diagnosis of either ‘classical’ autism, atypical autism or Asperger’s syndrome is now of a scale sufficient to make one ask whether autism is, in fact, exceptional. Only last month, there were newspaper headlines about the fact that about one per cent of schoolchildren in the UK are now recorded as having some kind of autistic spectrum disorder (double the figure from only five years previously). This was followed by the news that according to a new study by the Centres for Disease Control and Prevention, one in 88 children in the USA now have an autistic spectrum disorder (again, almost double the figure from five years previously).

We can speculate about the reasons for this recent upsurge, but, in order to understand it, I think it’s necessary to go back a little further and look at the broadening of autism through the concept of the ‘autism spectrum’, which is what has made it possible for autism to encompass high-functioning individuals such as myself. I think the potential for an unimpeded expansion of the category of autism, of the sort we are now seeing, may have already been there when autism was first conceptualised in the 1940s. It was certainly there once the notion of the ‘spectrum’ was introduced into psychiatry at the end of the 1960s.

I would argue that the category of autism has become less coherent, and consequently less meaningful and less useful, as a result of its expansion. And I think the osmosis into informal discourse and the pop culture of clinical terminology about autism has further undermined the category’s coherence. This has led to a situation where the ‘spectrum’ — once a categorical means of bringing together low- and high-functioning individuals who (arguably) have some features in common — is now routinely used to mean an uninterrupted continuum, ranging all the way from the pathological to the normal.

Autism may be linked to faulty mitochondria

A report in The Economist looks at a recent study which may explain the cause of autism:

One suggestion that does pop up from time to time is that the process which leads to autism involves faulty mitochondria. The mitochondria are a cell’s powerpacks. They disassemble sugar molecules and turn the energy thus liberated into a form that biochemical machinery can use. Mitochondrial faults could be caused by broken genes, by environmental effects, or by a combination of the two.

Nerve cells have a huge demand for energy, so a failure of the mitochondria would certainly affect them. The question is, could it cause autism? To try to find out Cecilia Giulivi of the University of California, Davis, and her colleagues studied the mitochondria of ten children, aged between two and five years, who had been diagnosed with autism. They have just published their results in the Journal of the American Medical Association.

The children in question were randomly selected from a previous study on autism. They were matched with ten children of similar ages and ethnic backgrounds who were developing normally. Dr Giulivi found that mitochondria from children with autism consumed far less oxygen than those from the control group. That is a sign of lower activity. One important set of enzymes — NADH oxidases — used, on average, only a third as much oxygen in autistic children as they did in non-autists, and eight of the autistic children had significantly lower NADH-oxidase activity than is normal.

The mitochondria of the autistic children also leaked damaging oxygen-rich chemicals such as hydrogen peroxide. These are a normal by-product of mitochondrial activity, but are usually mopped up by special enzymes before they can escape and cause harm — for instance, by damaging a cell’s DNA. The level of hydrogen peroxide in the cells of autistic children was twice that found in non-autists. Such high levels suggest the brains of autistic children are exposed to a lot of oxidative stress, something that would probably cause cumulative damage.

While such a mechanism may allow better treatments to be developed, it also implies that those who already suffer from autism may not benefit as much (or at all) from such treatments, as the cellular damage will be a much tougher challenge to reverse.

On the other hand, if the pattern can be detected early enough, it may allow treatment well in advance of serious damage.

The Lancet formally retracts controversial paper on Autism

In a long-overdue move, British medical journal The Lancet has retracted a paper by Andrew Wakefield on links between the MMR vaccine and Autism:

The Lancet medical journal formally retracted a paper on Tuesday that caused a 12-year international battle over links between the three-in-one childhood MMR vaccine and autism.

The paper, published in 1998 and written by British doctor Andrew Wakefield, suggested the combined measles, mumps and rubella (MMR) shot might be linked to autism and bowel disease.

His assertion, since widely discredited, caused one of the biggest medical rows in a generation and led to a steep drop in the number of vaccinations in the United States, Britain and other parts of Europe, prompting a rise in cases of measles.

The knock-on effect of parents avoiding getting their children vaccinated creates opportunities for much more serious outbreaks of these diseases. Dr. Wakefield’s “research” has been harmful to the population at large for helping to create and exacerbate parents’ fears for their children, and in encouraging them to take the greater risk of not getting the MMR (and, in many cases, other vaccinations) for their kids.
