Hm… I’ll take a stab at the connections.
Artist may be using the image of Stalin to show his liberal tendencies, therefore socialist. The Unicorn being a symbolism for that which Obama preached for his election campaign, the near impossibility of his words. Obama looks as many perceive him, a hero. Finally, Doctor House advocates the ideal of the means justifies the cause, the same way Obama is viewing a better America later, while making huge, crippling changes now.

Upon revision, I believe the Stalin holding a bottle of pills is symbolic of the possibility of the Universal Health Care which might be instituted in America.
